ABOUT US:

ABOUT THE JOB:

Reporting to the National Program Manager, Bounce Back the successful candidate will be responsible for coaching primary health care patients in a structured self-help program for anxiety and depression as part of a CMHA program called Bounce Back: Reclaim your Health. In addition, the position will liaise with primary care referrers to facilitate delivery of the Bounce Back program.

 

In this role, you will be supporting primary health care patients to acquire self-help strategies to manage anxiety and/or depression. In addition you will foster and maintain effective communication between CMHA BC Division, program referrers, Provincial Psychologist Consultants, and other local health organizations including CMHA Branches as required. This position also requires a continuous commitment to understand the role of primary health care in assisting people to manage their chronic health conditions.

 

Specific responsibilities include:

Patient coaching and support:

  • Provide guided self-help through telephone and/or group support based on a specified protocol to patients referred by their primary health care provider;
  • Provide motivational support and encouragement, reinforce workbook concepts and activities, and problem-solve to promote program completion;
  • Initiate and maintain a responsive, accessible service;
  • Report any patient concerns or safety issues immediately to the patient’s primary health care physician and the Provincial Psychologist Consultant;
  • Administer pre- and post–program structured questionnaires measuring patient’s quality of life and anxiety/depression symptoms;
  • Maintain basic patient records documenting progress through the self-help program and enter information in a timely manner into a secure database;
  • Maintain a flexible working schedule to ensure coaching services are offered at convenient times to the patient.

 

Liaison with primary care referrers and local community partners as required under the direction of the Program Manager:

  • Develop partnerships with community agencies/centres that could be involved in the dissemination/delivery of Bounce Back resources and services to support the needs of primary health care patients, including those with chronic health conditions ;
  • Develop communication and referral protocols with primary care practices participating in Bounce Back to ensure a responsive service;
  • Promote the Bounce Back program through participation in information sessions and other community events as required

ABOUT YOU

You will have a Bachelors Degree in a relevant discipline along with a minimum of 3 years of work related experience. An equivalent combination of education, training, and work experience may be considered. You will also bring to the job a serious interest in mental health, knowledge of the principles of cognitive-behavioural therapy, and the ability to relate to older adults with multiple health problems. Additional skills and experience are as follows:

 

  • Excellent communications skills
  • Basic computer literacy skills (e.g. MS Outlook, MS Word, Excel)
  • Familiarity through direct personal experience or the experience of family and friends in living with mood problems would be an asset
  • Oral and written fluency in French required

 

Working conditions

  • The work location is CMHA BC Division office in downtown Vancouver.
  • The usual workweek is 37.5 hours.
  • Flexible working hours is a requirement
  • Some limited travel is expected
  • Working in a variety of settings or in partnership with other health agencies may be required
  • Attendance at post-hire training events and participation at supervisory teleconferences is a requirement
